A Computer-aided Diagnosis Tool to Help the Treatment of Vascular Cerebral Accidents

Perda súbita de movimentos e de fala não precedidas por cefaléia ou outros sintomas de doenças evolutivas podem significar a ocorrência de AVC – Acidente Vascular Cerebral. Relatos médicos revelaram que o tratamento clínico dos AVCs é dependente do tipo de lesão ocorrida. No caso de lesões hemorrágicas o tratamento clínico deverá se basear na administração de medicamentos com funções de inibir o fluxo sanguíneo e priorizar a coagulação. Em contrapartida, se tratando de lesões isquêmicas, o tratamento clínico emergencial deverá ser fundamentado no aumento do fluxo sanguíneo com a administração de medicamentos anticoagulantes. O principal objetivo deste trabalho é a aplicação de técnicas de inteligência computacional e processamento de imagens na identificação de áreas hiperdensas e/ou hipodensas anormais em exames de Tomografia Computadorizada de Crânio em conjunto com a história clínica do paciente, para ajuda na decisão de procedimento emergencial para o tratamento clínico de AVC. As inovações propostas são a utilização da técnica de redes neurais com entradas baseadas nas anamneses coletadas, em conjunto com as imagens dos exames, para detecção da ocorrência de AVC hemorrágico ou isquêmico e a disponibilização de uma ferramenta amigável para ajuda no processo de decisão do tratamento clínico. Os objetivos deste trabalho foram alcançados. Foram obtidos valores de sensibilidade e especificidade de 96% e 94%, respectivamente, que demonstraram o bom desempenho do método e da ferramenta.

Abstract



Sudden loss of movements and speak non preceded by headache or other symptoms of evolutive diseases can indicate a Cerebral Vascular Accident – CVA. Medical reports revealed that the treatment of CVAs is dependant of the type of the lesion occurred. In case of hemorrhagic lesions, the clinical treatment must be based on the administration of medicines with the goal of inhibiting blood flow and priorizing coagulation. In the other hand, ischemic lesions require that the clinical treatment must be established considering the increasing of blood flow with the administration of anticoagulation medicines. The main objective of this work is the use of computational intelligence and image processing techniques in the identification of abnormal hyperdensive and/or hypodensive areas in Computed Tomography exams of the Brain together with the analysis of the patient’s clinical history, to help in the decision of the emergency procedure of the CVA treatment. The main innovations proposed in this work are the use of neural networks and image processing with inputs based on the collected anamnesis and the images of the exams, with the goal of detecting hemorrhagic or ischemic CVA and to make available to physicians a tool suitable to help in the decision of the clinical treatment. The targets of this work were reached. Sensitivity and specificity values were 96% and 94%, respectively, which showed the good performance of both the method and the tool.
